VitalLuxe IV is committed to protecting the privacy of your Protected Health Information (PHI) as required by the Health Insurance Portability and Accountability Act (HIPAA). We are required by law to maintain the privacy of your health information, provide you with this Notice of our privacy practices, and follow the terms of this Notice.

💊 How We May Use & Disclose Your Health Information

For Treatment: We use your health information to provide you with IV therapy services, assess your eligibility, customize your treatment, and coordinate with our supervising licensed Massachusetts physician.

For Operations: We may use your information for quality assurance, training, scheduling, and improving our services.

As Required by Law: We will disclose your health information when required by federal, state, or local law, including reporting requirements and law enforcement obligations.

For Public Health Activities: We may disclose your information to public health authorities to prevent or control disease, injury, or disability.

In Emergency Situations: We may share your health information to prevent a serious threat to your health or safety or the health or safety of others.

We will not use or disclose your health information for the following purposes without your written authorization:

  • Marketing purposes or sale of your PHI
  • Sharing with family members or friends (unless you authorize or it is an emergency)

  • Right to Confidential Communications: Request that we contact you in a specific way (e.g., only by email)
  • Right to a Paper Copy: Receive a paper copy of this Notice upon request

VitalLuxe IV implements physical, administrative, and technical safeguards to protect your health information from unauthorized access, use, or disclosure. All staff members are trained on HIPAA compliance and patient privacy.
